Unlikely, but...: ...something potentially serious is going on here. With abndominal pain, fever, and such a high WBC count, you need to keep working with your doctor(s) about the cause. With non-continuous pain, ruptured appendix isn't likely. But any number of other problems may be possible. If the pain is in lower abdomen, at age 21 pelvic inflammatory disease (PID, usually due to STD) is possible.
