Son just had a football physical the doctor said he had an enlarged level five lymph node what's our next step?

Hard to say: Since a "level 5" node is not a term commonly used in health care I would want an explanation from the source on what they meant. Perhaps they have a personal grading system, or the description was miss-characterized. Until you do that, there is nothing more to do.
