This is clearly: a serious problem. Both from a legal and a health standpoint. It is not possible to know if her feelings of unreality are from narcotic abuse or a separate psychiatric problem. It sounds like she likely needs to be assessed psychiatrically to determine if she needs some form of detoxification or rehabilitation. If she is arrested - it may force the issue.
