Deep venous thrombo: In DVT anticoagulation is started immediately IV heparin and oral coumadin till coumadin therapeutic or subcutaneous Lovenox . A work up to understand why the clot occurred is done that includes meds,pro S and C, ATT levels , antiphospholipid /anti cardiolipin ab, leiden factor, platelets, HCT level. Look for obstruction, or tumor - breast, lung ,colon . f/u w/ ER/ hemotology-today

Superficial clots.: The cephalic and basilic are both superficial veins and from whay you describe it appears that the clot did extend or more likely an additional clot has formed in the cephalic vein. This is most unusual unless you have had an iv or any drugs inserted into those veins. Initial treatment for superficial clots consists of heat to the area and ibuprofen. A clotting evaluation may be necessary.
