Ketonuria. : We can find ketones in the urine even if someone hasn't eaten in quite some time. Some people only eat one meal a day, and in between meals they'd fall into a starvation state.

Could be: hypoglycemia, ( low blood sugar), or may be your thyroid gland or other hormonal problem. Keep something sugary handy whenever you have the symptoms, like a candy, you need to see your doctor for evaluation and possible referral to an endocrinologist, good luck

Means nothing: Skip a meal or two and you'll get +1 ketones if the lab's doing a good job. If the weakness and shakiness are relieved by eating, consider postprandial syndrome or if they come on when you're fasting, a workup for insulinoma is in order -- there are many other possible causes. I hope several pathologists have examined the "autoimmune pancreatitis" biopsy -- if correct, you need specialist care.
