Back Pain and lump: Symptoms of a "knot" or "lump" are most likely a muscle spasm if painful. Understand that the spinal muscles attach from base of skull down to pelvis. So, spasms may travel the entire length in some cases. Shoulder pain/knot suggests levator scapula syndrome, which is a spasm of the muscle at the upper junction of shoulder blade and spine. Usually a posture problem. Try salonpas patches & stretch.
