if you have pain on your right side neck and shoulder can this cause pain on your right side lower back and down leg? no pain on left side at all

Yes: Neck sprains and these types of "spasms" can occur upon awakening and can radiate all the way to the feet. Also remember that neck and low back pain can result from the same sprain.
