I have right ovarian cyst size about 4.5 x 3.6 cm. 6 month ago i had a surgery of tumor in felopian tube and cyst in ovary. do i have to do surgery ?

Maybe not: Surgery may not be needed as small ovarian cysts can be sometimes suppressed by oral contraceptive pill usage. Talk to your MD about this as an option.
