Tdap injection: The deltoid muscle is the site of injection for Tdap and most other vaccines in adults. That area is chosen because there are no major veins or arteries in the vicinity. It should have been relatively obvious to the person injecting the shot if they hit a vein or not - so I really doubt this happened.
