Auto-inoculation: E coli is normal in our intestines; everyone has it. If in the throat, it usually gets there by auto-inocuation, i.e. someone's own hands from poor toilet hygiene, then touching the mouth or lips. Also more common in special circumstances, like being in the hospital, taking antibiotics, or immune suppression, e.g. by advanced cancer or other illnesses.
