Many possible causes: I assume he has been evaluated for strep, but he could also have a different bacterial infection (gonorrhea, fusobacterium), thrush, or a viral infection like coxsackie virus. Treatment depends on diagnosis. Some people get excellent relief with ibuprofen, salt water gargles, lozenges with benzocaine, or in some cases a steroid shot. Consider repeat in-person evaluation.
