Not always: Sometimes mild cystoceles don't bulge out unless you're bearing down. Severe cystoceles bulge out at rest. Sometimes it's an enterocele or rectocele that bulges out. You have to have a pelvic exam to really tell the difference between those three. Sometimes you can have all three bulging out.
