Now: If you are having inflammation around your ear due to bruxism, anti-inflammation will help right away. However, the medication is just temporary. The night guard appliance needs to be very balanced to your centric occlusion (ask your dentist) and not working occlusion, to balance your bite between the upper and lower teeth and relieve the strain. A dentist with advanced training in tmd is best.

Bruxism not ears: Bruxism does not cause an ear infection. Bruxism has nothing to do with an ear infection. You do not wear a mouth appliance for an ear infection. If you have or had an ear infection see a medical doctor as the dentist does not treat ears.
