Hello. i have been waiting for a family dr for months. i already have been diognost with anxiety disorder, are you willing to fill out disability form?

In-person exam: A thorough in-person evaluation, over the course of several visits, is needed in order to determine whether or not a person is disabled... and to determine what the disability means. For example, a person may be disabled in her own profession, or may be disabled in all professions. Also, the disability may be temporary (hopefully), or may be permanent. One can start with her primary care doctor.
