Thyroiditis: I suspect that you are being treated for Hashimoto's thyroiditis which has a nonspecific enlarged thyroid gland seen on ultrasound. Prominent cervical lymph nodes are not unusual in these cases but generally don't appear otherwise pathologic. Synthroid keeps your thyroid hormone levels normal. Your elevated PTH suggests a parathyroid adenoma which is best evaluated by an endocrinologist or surgeon
