Watchful waiting: A five year old with a cough and congestion for a few days can be watched at home and treated with fluids, humidifier and perhaps an OTC cough product (although they probably are not very effective). Your child should be seen by a doctor if there is wheezing, short of breath, fever, unable to drink fluids, or just "looks real sick" to you, especially if it lingers.

Viral URI: If the child is not having difficulty with breathing or spasms of cough that make him short of breath and does not have a fever, he most likely has a viral URI. This is usually a self limited infection and will resolve on its own. No cough medication or antibotics are needed, but if cough is bothersome, a tsp of honey mixed with lemon juice will calm the cough and may give some relief.
