Need details: Elderly women should not be treated for urinary infection unless they have obvious urinary symptoms, not for mental changes. Urine cultures are not reliable in this population because of incidental contamination of the specimen and long delay getting to the lab.

Maybe yes, maybe no: Frequently they work but the patient is 89 and you don't reveal any of her medical. If it doesn't clear up then you have to get a "clean catch" urine sample for a fresh culture and sensitivity and proceed from there.
