Knots: sounds like infection. As it is pain fun, likely to be boil or a tender Lymph node. will need antibiotics . See your doctor. If it doe snot settled down with a course of antibiotics then further investigations will be needed.

See a MD fast!: Painful skin masses can be due to boils (abscesses), which need treatment. When these cause surrounding redness and swelling, fever, or are increasing in size,then the infection is more serious. Get seen as soon as possible to be treated before the infection can get worse! If the pain, redness, swelling is rapidly worsening (noticeable spreading over hours, not days) get seen right away.
