What is a defect in the medial maxillary wall? it's in the paranasal but not sure where that is. i had sinus surgery to clear all sinuses last year.

Surgical?: I guess that this likely refers to the surgical procedure which widens up your maxillary sinus opening- which is desirable because it improves sinus drainage. If you have symptoms, it is best to contact the doctor who ordered the image study to find out its significance.
