Pregnancy test: It is possible to have an ectopic pregnancy but important to know that any pregnancy with a Mirena IUD is quite rare. The first step is a home pregnancy test. It would be positive even if an ectopic is present. Right lower abdomen pain if severe is concerning for appendicitis. Evaluation is needed

Get checked : Though the risk is low for pregnancy and you can't rely on menstrual cycles with mirena, you should get checked immediately if having right sharp belly pain. May also be your appendix. Seek immediate medical attention in an ER.
