Fungal infection: This sounds like onychomycosis, a non-dangerous infection, but one that discolors and deforms the nail. Debris can form under the nail, causing it to elevate. OTC meds are typically not very effective for this problem. Consult with your primary care doc or dermatologist to discuss treatment options.
