See doctor: It would be prudent to see a doctor as it is not feasible to provide a meaningful opinion without examination. Two issues come to mind, 1. Gout, 2 Diabetic lesion.

See a doctor: could be a fracture, could be infected, could be gout. Once an X-ray is taken a fracture can be ruled out. HE may need to be on antibiotics if an infection is present.
