Botox injection: See GI or colorectal surgeon to get Botox injections in anal sphincter. Relaxes muscle for ~ three months, breaking cycle of pain, spam, fissure that allows to heal while avoiding surgery. Topical nitroglycerin and calcium channel blocker (a blood pressure medicine) sometimes work as well. Botox seems to work better in my experience.
