High off and on?: AST is a sensitive test. But it also has small error rate such that a single value is not totally reliable. So it must be rechecked and confirmed. if two consecutive values are high, then it points to some liver cell damage taking place. Get it rechecked once more, in case it is causing you concern.

See answer: He should have his ultrasound repeated.IF he is obese(BMI =/> 30) the ultrasound is only 50% sensitive to detect fatty liver, i.e. half the patients with fatty liver won't be detected by ultrasound. Assuming his hepatitis B and C are normal other possibilities include hemochromatosis and Wilson's disease.
