Unlikely: Neuroleptic malignant syndrome has been rarely observed during treatment with promethazine. It usually occurs within the first 30 days after exposure to neuroleptics. One case noted in 1999: Neuroleptic malignant syndrome due to promethazine. Chan-Tack KM1. It occurred after 2 days in this case. If suspected it is an emergency situation.
