Non union?: This is a sign the bone has not healed which leaves the metal to fail in cyclical fatigue this failure is low energy event this is a common outcome of plating a clavicle fracture historically, symptomatic nonunion can be revised the asymptomatic loose screw can be ignored or followed to assure against gross migration of metal this sounds like a complex unstable high energy injury hope this help
