Let me explain: It sound to me you have bad case of gout in early age, you may have a family history of it. Also you have something else. My advise to you to see rheumatologist in your area to manage that condition and i hope you feel better soon.

Needs evaluation: If this is a chronic problem, it is more likely due to osteoarthritis. Have x rays done and see a rheumatologist.
