Could be either: Spotting prior to period on day 23 can occasionally occur prior to the onset of bleeding and not be a cause for worry, but there are other causes of irregular bleeding: pregnancy, fibroids, endometriosis, a cervical or uterine polyp. See your doctor to find out for sure.
