HYPERMETABOLIC MASS: The fact that you have this information reveals that you have had a study (or someone else has). The meaning of this test is that there is a mass that is very active metabolically--it uses a lot of energy--. This could be a hyperactive portion of the parotid gland or possibly a malignancy. It certainly needs to be followed and hopefully whoever ordered the test will follow up on it.

Possibly malignancy: Any solitary lesion of parotid should be resected with the possibity of malignancy. PET here being positive suggests lesion is cancer but low SUV with peak of 3.7 indicates lower grade malignancy. With possible deep lobe involvement, removing both lobes would require that at time of facial n. dissection that only ophthalmic and ramus be preserved.
