Orthopedic surgeon: You may not need surgery but you should have an evaluation by an orthopedic surgeon. Internal derangement of the knee can be torn cartilage or ligament damage. Any appropriate tests, like a MRI, will be ordered if deemed advisable. Often treatment consists only of rest, cold applications, anti-inflammatory medications and physical therapy.
