Not enough room: Your mouth is apparently too small for all of your teeth. Ask your orthodontist to explain. You will most likely be sent to an surgeon for the extractions.

Timing : Sometimes the orthodontist need to start the tooth movement as soon as teeth are removed. Sometimes without braces placed, patient may delay treatment after the extraction & causes abnormal shifting of the remaining teeth that can cause problems. As for orthodontist removing teeth, if they feel confident, I see no reason not to have them do it. Usually it's referred out.

Preference: Sequencing of treatment is up to your Orthodontic Specialist. Ask him/her why. Orthodontists like to talk about what we do and why. As to extractions, an Orthodontist does Orthodontics exclusively. If the practitioner who placed your braces also does fillings, cleanings, etc., he/she's not a qualified specialist. You will be referred to your Dentist or an Oral Surgeon for the extractions.
