Fluid retention: With progressive renal dysfunction, kidneys lose the ability to regulate salt and water balance. This can cause fluid retention & present as swelling in the legs, hands, feet, face and even the abdomen if severe. Diuretic medications which increase urination can help, but treatment should be directed by a nephrologist (kidney specialist) who understands the cause in these more advanced cases.
