Many things: Isolated, enlarged lymph nodes are relatively common. Just because the MRI (or worse, it's report) says something is "enlarged" doesn't mean it is pathological. Old infections & injuries, even healed, can leave scar tissue in the draining lymph node that persists for a lifetime. An FDG-PET scan is a better test to tell you if the node is active or not. "The purpose of a lymph node is to enlarge".
