Hard to say: Without visualization, it is hard to determine the cause. By now, your child should have an established relationship with a local dentist. If they don't, now is the time to start.

Cavity: Best guess would be cavity. 3yr old should have been seeing a Pediatric Dental Specialist or General Dentist since Age 1. Brushing 1x/day not enough. PLEASE take her to Dentist immediately. Don't delay.
