Multiple causes: The perineum, in women, is the area between the vagina and the anus.Common causes of pain include complications of childbirth, other trauma, abscess, entrapment of the pudendal nerve, nerve-associated cysts, pelvic inflammatory disease, ovarian cyst or torsion, even menstrual cramps. Inflammatory bowel disease/colitis can also cause. Requires careful evaluation usually by GYN specialist. Good luck
