Not paranoid: "Paranoia" means a delusion that you're being persecuted or that people are scheming against you. If you're not afraid that people are plotting to give you HIV, you're not paranoid. You don't live in a village in East Africa, you live in a 1st-World country with up-to-date medical systems. I'd be very surprised if they didn't observe Universal Precautions, including sterile single-use needles.
