Probable reflux: Reflux in adults is the second most common cause of chronic cough. The gag sensation could be from acid throat irritation while you are laying down. Simplest treatment would be no cigs, food or drink 4 hours before sleep and elevating head of bed to 30 deg. If no better then otc Omeprazole if not allergic twice daily for 2-3 months. If not better 4-6 weeks on Omeprazole then see md.

Hiatal hernia/colic: Biliary colic, hiatal hernia are possible diagnosis. The low functioning gallbladder by tech hida may be contributor to nausea. You are outpatient, so you have choice about next step in care. Take a friend with you to doc, ask questions about risk/benefit of ongoing medical care, further testing, adding surgical removal of gallbladder, nissan wrap. Good luck.
