Iv been having back pain, fatigue, decreased appetite, and slight swelling in my fingers and ankles. my labs came back and my bilirubin level is 2.3. should i be worried? my doctor wants to wait 3 months and see if it goes down before going further

Needs more info: Your age and symptoms are concerned for potential connective tissue/rheumatic condition other than Gallbladder issue with elevated bilirubin, such as ankylosis spondylitis for instance. I would advise to follow up with your PCP and possible rheumatologist if needed.
