See an immunologist: A 63 year old female with low IgG subclass 1 and elevated IgM, also with fatigue. A low IgG1 with normal total IgG could indicate a selective IgG subclass deficiency. But this value alone doesn't really tell us much. You need further evaluation by an immunologist to take a good history, learn about your infectious history, and obtain more loans - specifically response to vaccine challenge.

IgG1: IgG subclass includes IgG 1 to 4. IgG1 is the most predominant one. Its deficiency often leads to the susceptibility to recurrent bacterial infection. IgG deficiency can be caused by drugs, infection, asthma, or genetic defect. Sometimes, low IgG can be associated with elevated IgM. True IgG1 deficiency requires demonstrating low IgG1 level (<200) at 2 occasions and lack of response to vaccine.
