Varies: There are numerous potential causes, the three most common are usually an early degeneration on the medial aspect of the tibia, or a meniscal tear of the medial meniscus, or a tendonitis at the pea anserine area. A detailed exam and appropriate studies would likely reveal the cause, I would not hesitate to be seen.
