See below: For patients with a BMI > 30, who have failed to loose weight with diet and exercise, medication is advised. For patients with BMI >40 Bariatric surgery is advised. A BMI of 45 = extreme obesity. Surgery is advised if diet and exercise haven't helped. You need a comprehensive behavior modification program with graded exercise and specific diet. Orlistat should be considered.
