Radiculopathy: Although there are multiple potential causes, your symptoms are classic for a cervical radiculopathy, which is somewhat of a fancy way to describe a " pinched nerve" in the neck that can then cause pain or numbness down the arm. I would not hesitate to be seen, many good treatment options available.

Needs examination: That sounds terribly painful. Pins & needles sensation is often a sign of irritation to a nerve. As it is related to your sleep position it could be a nerve is getting cramped or pinched either in your neck, shoulder, or elbow. Some systemic problems can cause inflammation of nerves. It would be worthwhile to have a physical examination and perhaps a radiology study to determine the diagnosis.

If so severe: Get in touch with your physician, as you may well have a nerve compression at wrist, elbow, or even shoulder or neck. Imaging and an EMG will point the way.
