Infection: A cut resulting in a "black toe" is concerning for a serious infection. Have a doctor assess the lesion in person as soon as possible.

Varies: A diabetic who cut his toe and it is black likely has "gangrene" which is to say that the distal part of the toe has at least some degree of necrosis (dead tissue). Gangrene can either be dry (not associated with infection) or wet (infected). Safest thing would to be seen locally, placed on po antibiotics, and then quickly fly for followup unless not recommended by local doctor.

Could be bruise. Not: There is more going on here than meets the eye. Based on the description of the incident, the toe's vascular status has been compromised. This could either be by the trauma, an underlying vascular disease or both. His sugar control more than likely has influenced the gravity of the wound. The most effective time to begin treatment is immediately post injury, not after tissue death begins.
