Immediately: A normal person in a normal state of mind usually coughs up any food or gum in the windpipe immediately. A person who has some medical disability (unable to sense a piece of food there, or unable to cough effectively, etc.) might not cough it up, and end up choking or having pneumonia. A person abusing drugs or alcohol and not in a normal state of mind, may also end up choking or having pneumonia.
