Yes and yes: The most likely time for perforation is at insertion. However, in rarer occasions, IUD's have been known to "migrate"(perforate) during use. That is why a recommendation is to check your vaginal IUD string on occasion and especially if you are having cramping and pain to be certain the IUD didn't get "lost in space".
