Yes and yes: The most likely time for perforation is at insertion. However, in rarer occasions, IUD's have been known to "migrate"(perforate) during use. That is why a recommendation is to check your vaginal IUD string on occasion and especially if you are having cramping and pain to be certain the IUD didn't get "lost in space".

Uterus measures 6.6cm x 4.9cm on ultrasound. Which IUD would be the best fit: Mirena or Skyla?
