Cyst or lymph node: A soft lump behind the ear may be one of many things. A lymph node may remain enlarged after an infection. Any recurrent infections could make it tender. A cyst would be firmer, but attached to the skin. They can get tender when inflamed. Sounds like it should be evaluated by your surgeon.
