Complete ortho eval: You need a complete evaluation of your shoulder by an orthopaedic surgeon. There are several bursae in your shoulder that may become inflamed +/- connection with shoulder impingement syndrome or as a result of autoimmune disease, trauma, infection, or overuse. Rest and physical therapy with modalities such as ice, heat, taping, exercise, massage are first line. Steroid inj. not a panacea.

Therapy ? injection: In younger pts persistent pain very well could be rotator cuff tendinopathy and typically responds to therapy to restore range of motion, normalize mechanics of the shoulder blade and optimize cuff strength. If this fails to improve upon symptoms after 6-8 wks an injection in space above rotator cuff may be reasonable. If still no improvement obtain MRI to exclude surgical conditions.
