Yeast infection: Definitely yes - antibiotics are known to have side-effects and this is one of them. We call this Candidal vaginitis or a 'yeast infection'. I would definitely call your doctor to discuss. Perhaps there are other options for your acne...AND you want to get this cleared up. For a 'first' yeast infection, it's always good to get a doc's input (granted there are over the counter treatments).

Spironolactone: Spironolactone is not an antibiotic. It usually has no side effects. It is far less expensive and far better tolerated than doxycycline. For adult women is is far more effective for acne, Do you have normal menses? Use any birth control? If you use doxycycline, keep it to 20 or 50mg.

Yes, it could: Any antibiotic can potentially disturb vaginal flora. That's one of the reasons to take antibiotics with a probiotic supplement to prevent this type of side effect.
