Based on ankle exam: The degree of ankle sprain is classically based upon the degree of laxity and the area of focal tenderness upon physical examination. Laxity is derived by performing an anterior drawer test and a lateral tilt test to the ankle looking for tenderness to palpation and pain over the anterior talofibular ligament calcaneofibular ligament. A grade 1 sprain is injury but no rupture to the ligaments.

ANKLES AWAY!: Grade I-Stretching or slight tearing of a ligament with mild tenderness, swelling, and stiffness; walking possible w/ minimal pain. Grade II-Larger, incomplete ligament tear; moderate pain, swelling/bruising; walking painful; ankle "feels" stable. Grade III Complete ligament tear; severe swelling/bruising. Ankle is unstable; feels"wobbly"; walking usually not possible.
