Ligament damage: If we are referring to lateral ankle sprains (most common), the degrees are based on the number of ligaments damaged and/or torn. Each degree signifies more damage. 2nd degree indicates anterior tibial-fibular ligament and calcaneo-fibular ligament damage; 3rd degree includes the previous 2 as well as the posterior tibial-fibular ligament. It is important to have these sprains to be examined!

Ankle Sprain: A grade two or second degree sprain is associated with partial tearing of the injured ligament. A grade three sprain is a complete tear of the injured ligament and is associated with instability around the affected joint. Physiologically, a grade three injury creates instability at the joint whereas a grade two injury may remain stable. Grade 3 injuries often require surgical repair.
